Comprehensive Evaluation

First off, telling your health story.  You will complete several forms to assess your overall health.  This will allow you to include important details of your story with prompting questions. The goal being to leave no stone unturned.

Individual nutrition coaching / counseling

UNLIMITED one on one counseling sessions with our Bariatric Trained Health Care Professionals. The goal of these visits is to develop a personalized wellness plan based on your health history. The deliverables from these sessions include…individualized meal plans based on food preferences/intolerance, along with behavior and lifestyle recommendations / modifications.

Additional Testing Options

Additional testing may be helpful for you to accomplish your health goals.  Each test has individual pricing that is outside the cost of the membership fee.  All testing is optional and not required to be a member.

If further testing is needed, a personalized plan is developed from the test results and reviewed individually with the patient.
